Most of our forms are available below. Some of them are online forms that can be printed and completed, and some can be submitted electronically. We are striving to make all of our forms available electronically and available with electronic submission where possible. Please be patient as we continue the transition. Student Contracts are NOT listed online. These are issued by the school upon a student's acceptance to VIMSIA. For existing families, contracts are mailed spring of the prior year.

Any tuition that is not paid in full at the start of the school year, must be covered via automatic deduction or FACTS online. Use this form to indicate your preference.
Field Trip Authorization: Any student going on a field trip, must have a permission slip from their guardian(s). Some teachers have their own to send home, but this one will always suffice.
If interested in an Assistant Teaching position in our Montessori, L.A. Casa (Toddler), or Bougainvillea After School Enrichment program, please print out the form and return it to the school office, or fax to 775-3080.
If your child enrolls, he/she will be invited to choose a book which will be book plated with their name. They are the first to check it out, after which it is put into general circulation.
All students using Bougainvillea Enrichment Center for the afternoons on an ongoing basis are required to sign up for the program via this form. Please turn in to Ms. Paul, Ms. Phelan or Ms. Huttel
